Our main extended care center is where our long-term residents stay to receive a comprehensive care plan to foster the wellness of each and every resident. This allows us to maximize potential in an encouraging, positive, and dignified environment.

Respite care is available at the same cost as the daily rate. This service allows loved ones to take a needed break from their daily caregiving roles. The Office of Aging has a Medical Assistance waiver program available for respite care funding. We also accept funding from the Veterans Affairs Office and Hospice for respite care.
UnitedHealthcare Nursing Home Plan
The UnitedHealthcare Nursing Home Plan is a Medicare Advantage Special Needs Plan (I-SNP) that is customized with benefits specifically for long-term nursing home residents. The UnitedHealthcare Nursing Home Plan provides its members with an extra layer of care from a dedicated nurse practitioner who is in the facility regularly.
