The Social Services staff at Claremont is comprised of professionals working as part of a team, with other facility staff, to provide comprehensive care to our residents. A Social Services team member on each unit of our building is involved from the time of a resident's admission and will continue involvement throughout their stay.
Social Services Staff Responsibilities
The Social Worker/Social Service Coordinator provides:
Contact and support person for residents and family members
Ongoing support for adjustment to long-term care
Support service referrals in or out of the facility
On our short-term rehab unit, Transitions, the assigned Social Worker works closely with residents and family members to coordinate discharge when goals have been met. The Social Worker follows residents’ progress from the day of admission to the day of discharge, with an emphasis on coordinating a safe discharge to return to the community.
We individualize our services to meet the needs of each resident. Our Social Service team strives to assure that a resident’s stay at Claremont is comfortable, pleasant, and safe.
